In 2014, the North West AHP Workforce Board commissioned a project to review how AHPs across the North West are working differently to support service transformation. The project focussed on the frail, elderly population but the outcomes and learning from the project are transferable across other areas of AHP practice. A key output from the project was the case studies you can review below which demonstrate how AHPs are:
